By registering my child with Texas Bluejays F.C., I understand that participation in soccer practices, games, tournaments, training sessions, and other club activities involves normal risks associated with athletic activities, including falls, collisions, contact with other players, weather conditions, and possible injury.

I give permission for my child to participate in Texas Bluejays F.C. activities and confirm that, to the best of my knowledge, my child is physically able to participate.

I understand that Texas Bluejays F.C., its coaches, staff, volunteers, and representatives will make reasonable efforts to provide a safe and positive environment. However, injuries may occur during sports participation. I voluntarily accept the normal risks associated with my child’s participation and agree not to hold Texas Bluejays F.C., its coaches, staff, volunteers, or representatives responsible for injuries or losses resulting from the normal risks of participation, to the fullest extent permitted by law.

In the event of an injury or medical emergency, I authorize Texas Bluejays F.C. coaches or staff to contact emergency services and obtain reasonable emergency medical assistance for my child if I cannot be reached promptly.

I agree to inform Texas Bluejays F.C. of any medical condition, allergy, injury, medication, or other important health information that the coaching staff should know for my child’s safety.

By accepted below, I confirm that I have read and understand this waiver and voluntarily give permission for my child to participate in Texas Bluejays F.C. activities.
